Part-Time Experienced Practice Nurse Required
An exciting opportunity has arisen for a Practice Nurse to join Yetminster Health Centre. We are rural dispensing practice with approximately 4,300 patients in a small idyllic village just outside the abbey town of Sherborne.
We are looking for an enthusiastic, self-motivated, dynamic Practice Nurse to join our friendly multidisciplinary team for 22 hours per week (currently over 2 ½ days) Flexibility will be necessary to assist in covering annual leave and/or sickness.
The ideal candidates will have recent experience within a Primary Care Setting, be adaptable, highly motivated, relish a challenge and want to develop an ongoing career in Primary Care. We are a friendly practice who works closely as a team to deliver high standards of patient-centred care and have an excellent reputation with our mostly elderly patients.
You will be working alongside 4 GP partners, 1 other experienced Practice Nurse and 1 HCA, providing a whole range of practice nursing duties. The successful applicant should have good general practice experience in:
- Chronic Disease Management covering Asthma, COPD, Diabetes, Spirometry and all aspects of Practice Nurse treatment room duties
- Child & Adult Immunisations, Screening and Health Promotion
- Travel advice and vaccinations
- Cervical Cytology
- Family Planning/Sexual Health
- Dressings/Tissue Viability
The post holder will be responsible for the assessment of patient care needs and for planning, delivery and evaluation of programmes of care. They will provide a professional service performing all relevant forms of nursing care without direct supervision and providing support to the GPs. You will also be expected to take an active part in our monthly MDT meetings and Clinical Meetings to ensure ‘best practice’; make home visits as part of our frailty contract; complete clinical audits; assist in achieving QOF targets; organising annual Flu clinics and ensure that administrative tasks associated with clinical duties are complete. It would also be desirable for the post-holder to take on the Infection Prevention & Control for the practice.
